Financial & Insurance Information
We appreciate payment at the time of your visit. Payments can be made with cash, Mastercard or Visa and direct debit. *Please note: Payment is due at the time of service. As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.

Brightway Membership Program
Our Brightway Membership Program is available to any of our patients without dental insurance/discount plan. Avoid the hassles of insurance. No deductibles, waiting periods, annual maximum or denial of claims. Our membership program can provide an option for affordable routine care but also offers an additional discount for any restorative treatment you may need. This program is ideal if you are retired and no longer have dental coverage, do not receive dental insurance/benefits from an employer, and/or self fund your own dental care. What you get with the Brightside Membership Program:
Adult Standard
*This plan would be for patients with regular cleanings and absence of periodontal disease
Adult Periodontal
*This plan would be for patients that have been diagnosed with periodontal disease which requires additional care to maintain their oral health.
Youth Standard
*This plan would be for patients under 14 years of age.
*Procedures done outside our office are NOT included. No additional discounts apply. Discount does not apply to any products sold in the dental office. The annual fee is nonrefundable.
